The following FAQ is an overview of services provided by IMG. You should always consult plan documents for the most up to date information. When information here differs from plan documents, the plan documents will be followed.
How do I enroll in an IMG medical insurance plan?
To enroll in medical insurance contact Human Resources. All Global Outreach W2 missionaries are required to have medical insurance with either Highmark BCBS or IMG. Exceptions include any family that has Medicare, Tricare, or Medicaid.

What does my insurance cover?
Your insurance coverage is summarized in the SPD booklet file below. Outside of the United States the plan will pay 100% of most of your medical costs. This is subject to all the plan policies and provisions. In the United States the plan uses the United Healthcare PPO network. While there is a $25 copay for basic doctor appointments and a tiered copay system for prescriptions, there is a $4500 deductible which covers many types of tests and procedures. You should refer to the SPD Booklet carefully for plan information.

Why should I choose to obtain Healthcare outside of the United States?
Healthcare outside of the US in many places is as good or better than what you may receive in the US. Obtaining healthcare outside of the country saves money for missionaries and for Global Outreach. You do not have to return to the US to receive treatment which can save money and time for missionaries. If you stay in an overseas hospital overnight you may receive an indemnity payment for each night that you are there. See the SPD for plan maximums. IMG has a Medical Concierge service that can help you get treatment in areas with medical services if you are in an area where you cannot receive adequate medical service.

What is a Direct Billing Provider and how do I find one?
When you search on the IMG website referenced above, you will be given a list of providers that IMG has used in the past. You are not restricted to only the providers on this list. In this list you will see that some providers have a yellow star beside their listing. These are direct billing providers. This designation should mean that the provider will not demand payment upfront and will accept payment from IMG directly. You will still need to proceed through any pre-certifications or pre-authorizations you are required to perform. Contact IMG using the Customer Service information on your ID card or on the website.

When do I need to get a pre-certification completed?
You must consult your SPD, downloadable below, to know when you need a pre-certification or a pre-authorization from IMG for a procedure. You should contact the customer service # on your ID card to initiate this process.
You should be advised that any invasive procedure, including procedures ending in -ectomy, otomy, -scopy, biopsy, requires pre-certification, including diagnostic or outpatient procedures. There are many surgical procedures, and we can’t list them all in the certificate. We advise members to err on the side of caution and contact IMG for pre-certification of any invasive procedure. You will be advised if pre-certification is not required.

The baby must be added to your insurance within 30 days of the birth. You will need to contact Human Resources to initiate this process. Failure to get the proper enrollment forms complete and submitted to the insurance company before the 30 day period may result in reduce coverage benefits for the delivery and will subject the child to a pre-existing conditions clause. What do I do if I am having problems using my insurance? Who do I contact when I have questions related to insurance?
If you have general questions about your coverage or you are having issues with using your IMG insurance, you should first contact IMG Customer Service using the contact phone number on your ID card or on the website. If IMG does not resolve your issue please contact Human Resources for additional assistance.

What is an EOB or Explanation of Benefits notification?
An EOB , Explanation of Benefits, explains the details about a claim that has been submitted to BCBS. It tells what is covered and what might be the patient's responsibility. If you receive an EOB stating that you owe an amount, wait until you receive a bill from the provider before you pay the amount you are responsible for. What does it mean if I get an EOB stating that my claim is being applied to my deductible and that I am responsible for payment. I thought IMG paid for everything overseas.
Insurance can be very difficult to understand. Our plan can be doubly confusing. Here is why:
Each missionary actually has a $4000 deductible they must meet before the insurance plan pays anything. The question then becomes, if that is so then how is everything covered at 100%. Global Outreach has a HRA account that pays the deductible for the missionary. Therefore effectively giving you $0 that you are responsible for in most cases. The insurance processes that in 2 steps. First it processes the claim you submit under the plan taking into account the $4000 deductible. That is why you will get an EOB that says an amount is not covered and is your responsibility to pay. Then the insurance company processes it again against the HRA account. When that happens you get reimbursed from Global's self funded HRA account. Once you reach the $4000 deductible it no longer does the second processing step. You get paid directly from the plan on the first processing of the claim.
You must look at your claims and see if it is processed and then paid in two different steps. If you ever have questions about this you can contact Human Resources.

IMG pays an indemnity payment to members when they stay overnight in an international hospital. When staying at a private international hospital the plan pays $400 per night up to a maximum of $4,000. When staying at a public hospital the plan pays $800 per night up to a maximum of $8,000. A public hospital is typically sponsored by the state or a charity and does not present charges for treatment to the patient or to IMG.

Missionaries who will be on extended furlough or who are traveling domestically for more than 3 months should contact Human Resources. You will be moved to the Highmark BCBS plan administered by Guidestone. This plan is suited for domestic use.
